One of those cost drivers now has a federal deadline behind part of it. The CMS interoperability and prior authorization rule (CMS-0057-F, 89 FR 8758, published February 8, 2024) requires its impacted payers other than exchange plans, meaning Medicare Advantage plans and Medicaid and CHIP fee-for-service and managed care plans, to decide prior authorization requests within 72 hours for expedited and 7 calendar days for standard requests, and requires all of them to give a specific reason for a denial, with the timelines starting January 1, 2026. Commercial carve-outs are outside that rule, so the authorization workload is uneven across a BH payer mix. For broader outsourced-billing context, see the medical billing cost guide, and percentage vs flat-fee medical billing is the model-choice comparison.
The Medicare piece of a solo practice's payer mix changed recently. The CY 2024 Physician Fee Schedule final rule (CMS-1784-F, 88 FR 78818, published November 16, 2023, effective January 1, 2024) implemented Medicare enrollment and billing for marriage and family therapists and mental health counselors, so an LMFT or LPC who could not bill Medicare before 2024 now can, which adds a payer the biller has to know. The HCPCS codes the SUD answer below relies on are maintained in CMS's quarterly HCPCS update files, most recently the October 2026 file. Before signing, run the vendor through the questions to ask a medical billing company.
A behavioral health carve-out is when a commercial insurer routes mental health and SUD claims to a separate managed behavioral health organization rather than processing them under the parent medical plan. The largest BH carve-outs are Magellan, Carelon (formerly Beacon), and Optum BH. Each maintains its own provider network, prior authorization rules, fee schedules, and submission portals. A biller that does not handle carve-out routing will misroute claims, miss prior auth windows, and create AR aging problems specific to BH payer mix.
A carve-out does not get its own rulebook. Under the federal parity regulation at 45 CFR 146.136(c)(4), medical management standards such as prior authorization are nonquantitative treatment limitations that a plan may not apply to mental health or substance use disorder benefits more stringently than to medical and surgical benefits, and that holds whether the plan processes behavioral claims itself or routes them to a managed behavioral health organization. A biller with real carve-out experience knows which authorization patterns are worth a parity challenge. For the practice's own enrollment with those carve-outs, credentialing for mental health practices covers the paneling side.
Placement on this page cannot be bought. Vendors are included using our published methodology and the information and evidence available for each vendor. You can submit corrections or supporting evidence for review. Providers are listed alphabetically. Paid placement is labeled separately.
Operator note: if your treatment center runs as a multi-site organization with payer-mix and competitive-positioning questions on top of billing, the billing vendor decision is downstream of broader operational visibility. Our colleagues at GetTreatmentHelp publish operator-level analysis on multi-site evaluation at GetTreatmentHelp's operator briefing center, which pairs naturally with the BH billing decision covered here.
