How we tested

Use the linked vendor records to check documented terms. Ask providers to demonstrate your required workflows and confirm current commercial terms in writing.

Guide inclusion criteria explain the evidence-packet policy and current guide orders. Full methodology is published at our Buyer's Guide methodology page.

How to choose EHR software for primary care offices

  1. Pricing model fit — Decide whether per-provider flat pricing or percent-of-collections matches your panel economics. A growing PCP group with rising collections often pays more under percent-of-collections; a high-volume Medicare practice with thin margins per visit may pay less.
  2. MIPS and quality-program alignment — Confirm built-in MACRA/MIPS reporting and care-gap surfacing. For Medicare-heavy primary care panels, this is the difference between a positive and negative payment adjustment, often worth more than the EHR fee itself.
  3. Charting speed for the primary care visit cadence — Time five common visit types in the demo: annual wellness visit, hypertension follow-up, diabetes follow-up, acute upper respiratory, and Medicare ACO visit. The platform that's two minutes faster per visit gives a 20-patient PCP back 40 minutes a day.
  4. Billing and RCM depth — If you bill insurance, verify integrated claims, denials management, and eligibility checks. The "EHR plus separate billing tool" stack tends to cost more in reconciliation labor than one integrated suite.
  5. Support quality under stress — Ask references about average ticket resolution time during claim-rejection surges and Medicare rule-change quarters. Mid-pack support that handles January and October well beats premium support that doesn't.

One more line item for a primary care office: electronic prescribing of controlled substances. Under 42 CFR 423.160(a)(5) prescribers must send at least 70 percent of their Part D controlled-substance prescriptions electronically, with compliance actions beginning January 1, 2023 and an exemption for prescribers who write 100 or fewer such prescriptions a year, so EPCS should be confirmed as included rather than assumed. For the pricing-model question in item 1, the EHR pricing guide carries the per-provider and percent-of-collections ranges, and the EHR vendor question list is the demo script.

Frequently asked questions

What does EHR software actually cost for a primary care office in 2026?

3 of the seven vendors compared on this page publish a per-provider monthly price, spanning $126-$499 per provider per month (base-plan price per vendor); the other 4 (Elation Health, athenahealth, Tebra and DrChrono) are quote-only. Sources, each checked 2026-10-05: RXNT pricing, Practice Fusion pricing, eClinicalWorks pricing.

Does the EHR I pick affect MIPS scoring and Medicare quality payments?

Evaluate the EHR against the quality measures your practice reports. Confirm the reporting workflow during a demo.

The stakes are set in regulation. Under 42 CFR 414.1405(c) the MIPS payment adjustment is built on an applicable percent of 9 percent for MIPS payment year 2022 and each later year, with the adjustment factor set by comparing a clinician's final score to the performance threshold, and the CY 2026 Physician Fee Schedule final rule (CMS-1832-F, 90 FR 49266, published November 5, 2025, effective January 1, 2026) keeps that threshold at 75 points for the CY 2026 through CY 2028 performance periods. An EHR that surfaces the measures during the visit is working against a fixed bar, which is why the fee difference between platforms is small next to the adjustment on a Medicare-heavy panel.

Can I switch EHRs without losing patient chart data?

You can, but expect tradeoffs. Most vendors will migrate structured data — demographics, problem lists, medications, allergies, immunizations, recent labs — through their onboarding team. Free-text notes and scanned documents are messier; some practices keep read-only access to the legacy EHR for one to two years rather than migrate every PDF. Negotiate the migration scope and the legacy-access timeline before signing, and ask the new vendor about supported import formats (CCDA, HL7, flat-file) for your current platform.

The import formats have a federal floor. ONC's certification criteria at 45 CFR 170.315(b)(1) require a certified EHR to send and receive transition-of-care summaries, and 45 CFR 170.205(a)(4) names the HL7 Consolidated CDA Release 2.1 templates as the standard for those documents, so a C-CDA export of structured data is something every certified vendor can produce on request. Free text and scanned documents sit outside that floor, which is where the negotiation happens. The EHR migration guide walks the cutover, and the EHR contract clauses that cost practices money lists the terms to fix before the legacy vendor has leverage.

How do vendors get included?

Placement on this page cannot be bought. Vendors are included using our published methodology and the information and evidence available for each vendor. You can submit corrections or supporting evidence for review. Providers are listed alphabetically. Paid placement is labeled separately.
