How we tested

Use the linked vendor records to check documented terms. Ask providers to demonstrate your required workflows and confirm current commercial terms in writing.

Guide inclusion criteria explain the evidence-packet policy and current guide orders. Full methodology is published at our Buyer's Guide methodology page.

How to choose practice management software for physical therapy clinics

  1. HEP adherence as a clinical KPI. Ask how home exercise programs are authored, delivered and measured.
  2. Total cost of ownership across add-ons — Headline per-provider rates are misleading. Add billing modules, engagement tools, telehealth, SMS, and per-practitioner Insurance Billing fees before comparing. The cheapest published price rarely stays cheapest after eight months of adoption.

Three of the criteria above rest on Medicare rules a PT-specific platform has to encode correctly, and the rules are published. The 8-minute rule is Medicare Claims Processing Manual, Chapter 5, section 20.2 (Rev. 11129, November 22, 2021): a single timed code in 15-minute units bills one unit at 8 through 22 minutes and two units at 23 through 37, and a lone service under 8 minutes is not billed at all. The KX modifier threshold is set each year on CMS's Therapy Services page (last modified March 10, 2026): $2,480 for CY 2026 for PT and SLP services combined, with claims over that amount denied without the modifier, and a separate $3,000 targeted medical review threshold that holds through 2028. The same page sets the assistant rule a visit-tracking module also has to handle: services furnished in whole or in part by a physical therapist assistant have been paid at 85 percent of the fee schedule amount since January 1, 2022 and carry the CQ modifier when the assistant's independent portion exceeds 10 percent of the service. Plan-of-care certification is the fourth: 42 CFR 424.24(c) requires recertification at least every 90 days, which is the interval an authorization alert should be counting toward.

The platform decision sits inside two larger ones. Whether a PT clinic needs a separate EHR at all, or a practice management system with rehab documentation is the whole stack, is the question in practice management software vs EHR; and clearinghouse selection is covered in how to choose a clearinghouse for an independent practice. Clinics weighing whether to keep billing in-house on any of these platforms or hand it to a vendor should run the numbers in outsourced vs in-house billing, and the add-on arithmetic in the last criterion is the same one the EHR pricing and cost guide works through for general-purpose systems.
